7 Conditions You Can Treat at a Pharmacy Without Seeing a GP

Getting a GP appointment can take weeks these days. But what if I told you there are seven common health conditions you can have treated at a pharmacy — with prescription-strength medication, no doctor's letter needed?

Since January 2024, NHS Pharmacy First has made it possible for pharmacists in Wakefield and across England to diagnose and treat specific conditions. You walk in, describe your symptoms to a trained pharmacist, and if you meet the criteria, you walk out with treatment the same day. No appointment, no waiting.

This is how Pharmacy First conditions work, and it's changing how accessible healthcare is for Wakefield residents.

Sore Throat

A sore throat is one of the most common reasons people used to book a GP appointment, yet most resolve on their own or respond well to pharmacy treatment. Through Pharmacy First, your pharmacist can assess anyone aged 5 and over, ask about symptom severity and duration, and examine your throat to rule out anything that needs a doctor.

If they confirm your sore throat fits the criteria — typically a bacterial throat infection — they can provide antibiotics on the spot. If they suspect something more serious, like quinsy or a persistent infection needing specialist input, they'll refer you to your GP or urgent care.

Sinusitis

That blocked, heavy feeling across your forehead and sinuses is often sinusitis — inflammation of the air-filled cavities around your nose. Pharmacy First covers sinusitis for patients aged 12 and over.

Your pharmacist will ask how long you've had it, whether it started after a cold, and what you've already tried. Most cases clear up on their own, but if you meet the criteria, your pharmacist can prescribe antibiotics to help ease the infection and discomfort. If symptoms have lasted months, there's high fever, or severe pain, they may suggest you see your GP instead.

Earache

Earache is particularly distressing — the pain can be sharp, dull, or throbbing, and it's hard to ignore. Pharmacy First covers earache in children and young people aged 1 to 17, which is especially valuable since GP waiting times for paediatric issues can be long.

Your pharmacist will ask about pain intensity, duration, whether there's discharge, and whether recent colds or ear infections came before it. They'll advise whether treatment is needed immediately or whether it's safe to monitor at home. If pain is severe, the ear is discharging pus, or there are signs of infection, they can prescribe antibiotics or ear drops.

Infected Insect Bites

An insect bite that becomes red, swollen, warm, or weepy — especially one that won't heal — may have become infected. This happens more often in summer or after trips. Pharmacy First covers infected insect bites for patients aged 1 and over.

Your pharmacist will examine the bite, look for signs of infection, and provide treatment if needed. They can also advise on home care and which warning signs mean you should see a doctor: spreading infection, fever, or large swelling.

Impetigo

Impetigo is a skin infection that causes sores, blisters, or crusts, usually around the nose, mouth, or hands. It's highly contagious and common in young children, though it can affect anyone. Pharmacy First covers impetigo for patients aged 1 and over.

If your pharmacist confirms symptoms match impetigo — typically shiny, honey-coloured crusts or fluid-filled blisters — they can prescribe antibiotic cream or antibiotics by mouth. They'll advise on hygiene: keep the area clean, avoid touching it, and wash hands frequently to prevent spreading to family members.

Shingles

Shingles is reactivation of the chickenpox virus, and it causes a painful, blistering rash usually appearing on one side of the body. It's most common in adults over 50, though it can occur at any age. Pharmacy First covers shingles for adults aged 18 and over.

Early treatment matters — antivirals work best within 72 hours of the rash appearing. Your pharmacist will assess the rash, ask how long you've had it, and if you meet the criteria, can provide antivirals straight away. Learn more about managing shingles on the NHS website.

Urinary Tract Infections (UTIs)

UTIs are extremely common, particularly in women. Symptoms include a burning sensation when passing urine, needing to go more often, and lower abdominal discomfort. Pharmacy First covers uncomplicated UTIs in women aged 16 to 64.

Your pharmacist will ask about your symptoms and history. If your symptoms fit the criteria for an uncomplicated UTI, they can prescribe antibiotics without a GP appointment. If you have fever, kidney pain, blood in your urine, or symptoms outside the standard pattern, they'll refer you for investigation.

How Pharmacy First Works in Practice

The process is straightforward.

Walk in — no appointment needed. Visit Kingfisher Pharmacy during opening hours and tell reception you'd like a Pharmacy First consultation.

Speak with a pharmacist in private — you'll have a confidential discussion in a consultation room. The pharmacist will ask about your symptoms, how long you've had them, what you've already tried, and relevant medical history.

Get assessed — the pharmacist may examine the affected area (throat, ear, skin) or ask additional questions to rule out conditions outside Pharmacy First's scope.

Receive treatment — if you meet the criteria, you'll get prescription-strength medication on the spot. Free if you're eligible for NHS prescriptions; otherwise, standard prescription charge applies. If your condition falls outside Pharmacy First, the pharmacist will refer you to your GP or urgent care and explain why.

Get aftercare advice — the pharmacist will explain how to take your medication, what side effects to watch for, and when to return if symptoms don't improve.

Most visits take 10–15 minutes. You don't need to book ahead or get a referral from your GP.

Why This Matters for Wakefield Residents

GP waiting times in Wakefield can stretch to several weeks for routine appointments. Pharmacy First changes that equation for common conditions.

Instead of waiting to see a doctor, you get expert assessment and treatment from a registered pharmacist within the same day. Pharmacists train for a minimum of four years in medicine and pharmacology — they're highly qualified to diagnose these specific conditions and provide treatment.

It also means GPs have more capacity for complex cases and chronic disease management — the things that really need a doctor's time. Learn more about how Pharmacy First is reducing GP waiting times in our full guide.

When to Seek Other Care

Pharmacy First is brilliant for the conditions it covers, but there are times to seek other care.

  • Fever with a rash: Could be meningitis or another serious infection — go to A&E or call 999.
  • Severe pain: If pain is unbearable, call 111 or go to urgent care.
  • Symptoms lasting months: Chronic issues often need investigation, not just treatment.
  • Complex medical history: If you have diabetes, heart disease, or take multiple medications, your GP is better placed to assess whether Pharmacy First is safe for you.
  • Pregnancy: Pregnant women should speak to their GP first before using Pharmacy First.

Not sure whether your symptoms qualify? Call 111 or visit Kingfisher Pharmacy and ask — we won't send you away if we think you need a doctor. Find out more about when to use Pharmacy First instead of booking a GP appointment, or discover when to visit your pharmacy versus A&E.

Frequently Asked Questions

Can I use Pharmacy First if I'm not registered with a Wakefield GP? Yes. Pharmacy First is available to anyone with NHS coverage. You don't need to be registered with a local practice, though most Wakefield residents are.

What's the cost? If you're eligible for free NHS prescriptions (under 16, over 60, pregnant, certain medical conditions, or low income), you pay nothing. Otherwise, you'll pay the standard prescription charge.

What if I don't meet the criteria? If your condition is outside Pharmacy First but doesn't seem urgent, your pharmacist may advise you to wait and monitor at home, or suggest over-the-counter options. If they think you need a doctor, they'll explain why and help you understand your next steps.

Can I go back to the same pharmacy if the condition comes back? Yes. You can use Pharmacy First for repeat episodes of the same condition, as long as you meet the criteria each time. Read more about repeat Pharmacy First visits.

Do I need my GP's permission? No. Pharmacy First is independent — you don't need a referral or letter from your doctor.

What if I'm worried it's something more serious? Tell your pharmacist. They're trained to spot red flags and will refer you if something needs a doctor's attention. Never feel pressured to accept pharmacy treatment if you're unsure.

Can children use Pharmacy First? Some conditions are available for children aged 1 and over (infected insect bites, impetigo, earache). Sore throat is available from age 5. Read our parent's guide to Pharmacy First for more details.

If my condition doesn't fit Pharmacy First, do I have other options? Yes. Over-the-counter treatments may help, or if it's urgent (but not A&E-urgent), you can call 111 for advice. Your pharmacist can also help you book a GP appointment if needed.
